Precautions for negative pressure aspiration during abortion

Vacuum aspiration can be performed under local or general anesthesia, and can be performed with a metal dilator, seaweed band, or 400 pg of misoprostol given orally or vaginally 3-4 hours before the procedure to dilate the cervix. The diameter of the suction tube is 5-12 mm, and the negative pressure should not exceed 500 mmHg. For early pregnancy abortion, the artificial negative pressure generated by a 50 ml syringe is equivalent to the negative pressure generated by electric aspiration.

1. Contraindications: genital tract inflammation, pelvic inflammatory disease, various acute diseases or acute infectious diseases, heart failure, hypertension with subjective symptoms, acute stage of tuberculosis, severe anemia, high fever or body temperature >37.5℃ twice on the day of surgery, etc.

2. Preoperative Preparation

(1) Detailed medical history, bimanual gynecological examination, urine HCG and B-ultrasound examination were performed to confirm intrauterine pregnancy.

(2) Routine vaginal discharge and blood tests should be performed to determine whether there is inflammation, anemia, and thrombocytopenia. Patients with missed abortion should also have their coagulation function tested.

(3) Measure body temperature, pulse, blood pressure, and perform routine medical examinations.

(4) Inform the patient of the surgical process and possible consequences to relieve the patient’s concerns and strengthen contraceptive and health education.

3. Analgesia and anesthesia The commonly used anesthesia method for painless abortion in early pregnancy is intravenous administration of propofol anesthesia. Propofol, chemical name: 2,6-dipropofol, is a new short-acting intravenous general anesthetic. Its clinical characteristics are rapid onset, short duration, rapid and stable awakening, and few adverse reactions. It is suitable for inducing or maintaining general anesthesia, auxiliary medication for spinal anesthesia, patient-controlled analgesia, and minor outpatient surgery. Because propofol has the effect of inhibiting the vagus nerve reflex, it can significantly block the vagus nerve excitement caused by cervical dilation, thereby reducing the occurrence of abortion syndrome.
