If it grows outside this range and invades the myometrium, it becomes a pathological condition called adenomyosis. If the lesion is confined to one site, it becomes an adenomyoma. Symptoms of adenomyosis 1. Dysmenorrhea Most cases are secondary dysmenorrhea with progressive aggravation, and the condition is severe, often requiring pain relief. As adenomyosis develops, pain may begin about 1 week before menstruation, or extend to 1 to 2 weeks after menstruation. A small number of patients with adenomyosis still have cyclical pain before and after menstruation. 2. Dysmenorrhea Among the 318 cases reported in China, 132 (41.5%) had menstruation. A small number of patients with adenomyosis had heavy bleeding before surgery, which was misdiagnosed as functional uterine bleeding. 3. Infertility There are many symptoms of adenomyosis, and infertile patients are a minority of patients with adenomyosis. 4. Enlarged uterus Most of them are uniform and hard, and their size is generally no more than 12 weeks. Otherwise, they may be related to uterine fibroids. If it is adenomyosis, it may also show asymmetrical enlargement. If combined with endometriosis, there may be corresponding signs. Treatment of adenomyosis (1) Symptomatic treatment For patients with mild symptoms who only need to relieve dysmenorrhea, non-steroidal anti-inflammatory drugs such as ibuprofen, indomethacin or naproxen can be chosen for symptomatic treatment. (2) Sham treatment For patients with mild symptoms, no fertility requirements, and approaching menopause, oral contraceptives or progesterone can control the development of adenomyosis by causing decidualization and atrophy of the ectopic endometrium. (3) Intrauterine device For those who have heavy menstrual flow, dysmenorrhea, and do not want to have children, a highly effective progesterone contraceptive device can be used. The continuous release of progesterone in the uterus can control the development of ectopic lesions. It needs to be removed or replaced after five years. (4) Treatment of pseudomenopause Drugs that reduce preoperative lesions and postoperative recurrence. GNRHA injection can make the hormone level in the body reach the menopausal state, gradually shrink the ectopic endometrium and play a therapeutic role. GNRHA can significantly reduce the size of the uterus and can be used as a preoperative medication for some patients with large lesions and difficult operations. When the uterus becomes smaller before surgery, the risk and difficulty will be greatly reduced. Patients with adenomyosis may have symptoms such as abnormal menstruation, intercourse pain, pelvic pain, low-grade fever, increased vaginal discharge, chronic pain, etc. For those with mild symptoms who only need to relieve dysmenorrhea, non-steroidal anti-inflammatory drugs such as ibuprofen, indomethacin or naproxen can be used for symptomatic treatment. |
