How to detect threatened abortion:

Ultrasound examination: Through vaginal ultrasound examination, the gestational sac can be seen about 35 days after the last menstrual period, and the primitive heart tube pulsation of the fetus can be seen after 42 days; through abdominal ultrasound, the gestational sac can be seen 35 to 42 days after the last menstrual period, and the primitive heart tube pulsation of the fetus can be seen 49 days after the last menstrual period. For those suspected of threatened abortion, the survival of the embryo or fetus can be determined based on the presence, shape, position, fetal heart reflex and fetal movement of the gestational sac. If the embryo stops developing, the gestational sac will be deformed, the embryo will wither, and the fetal heart pulsation will disappear. Incomplete abortion and overdue abortion can be determined with the help of ultrasound.

Urine pregnancy test or determination of β-subunit levels in the blood: Positive urine pregnancy test: At 33 to 35 days after menstruation, when the gestational sac is seen by vaginal ultrasound, the subunit level in the blood is about 1800 milliunits/ml, and can reach 20,000 units/ml for a period of time. Values ​​below this generally indicate a poor prognosis.

Blood progesterone level measurement: Recent studies have found that the measurement of blood progesterone levels is more accurate in judging the prognosis of pregnancy. If the blood progesterone level is lower than 5 ng/ml, whether it is an intrauterine pregnancy or an ectopic pregnancy, it generally indicates that the pregnancy is dead. If the blood progesterone level is higher than 25 ng/ml, it indicates that the intrauterine pregnancy is alive; if it is between 5 and 25 ng/ml, reexamination and follow-up are required.

Gynecological examination: The doctor needs to use a speculum to do a vaginal examination. Don't worry, this will not increase the risk of miscarriage. The main purpose is to check the amount, color, and source of bleeding, as well as to check whether there are any lesions in the vagina and cervix, whether the cervical opening is dilated, whether there is any tissue stuck in the cervical opening, whether the size of the uterus is consistent with the number of weeks of amenorrhea, whether there is any tenderness, the shape of the uterus and whether there is any mass or tenderness in the bilateral adnexal areas, etc.
