The Bartholin's glands are located at the back of the labia majora on both sides, and the gland duct opens to the inner side of the labia minora near the hymen. Treatment of Bartholinitis

1. Once an abscess is formed, the use of drugs alone is ineffective and it should be incised and drained. The timing of the operation should be chosen when the fluctuation is most obvious. Generally, the incision is made below the inner side of the labia majora. The incision should not be too small so that all the pus can be completely drained out. After the pus is drained and the inflammation begins to subside, use 1/5000 potassium permanganate solution for a sitz bath.

2. In the early stage of Bartholinitis, systemic treatment can be used. Since Bartholinitis caused by gonococci has been increasing in recent years, it is best to squeeze the urethral opening before taking the medicine, or take the cervical canal secretions for bacterial culture and do bacterial sensitivity test. Before the results of the sensitivity test come out, choose the medicine based on experience. Generally speaking, penicillin drugs have better efficacy, but local hot compresses or physical therapy should also be used according to the situation to promote the disappearance of inflammation. At the same time, the vulva should be kept clean and hygienic, and chemical fiber underwear should not be worn.

3. During the acute stage of vestibular glanditis, you should absolutely stay in bed and rest, pay attention to local cleaning, local cold compress, and apply antibiotics. If an abscess has formed, it should be incised and drained immediately. The incision should be made at the thinnest part of the skin. Generally, a half arc incision is made on the inner side of the labia majora to drain pus. You can also use an 18-gauge needle to pierce the abscess cavity from the mucosal side after disinfecting the vulva, suck out the pus, leave the needle in place, and slowly inject 200,000 to 400,000 units of penicillin saline. After pulling out the needle, press with gauze or cotton balls for several minutes to prevent the drug from leaking out, and add a vulvar pad and fix it with a menstrual belt. After this method of treatment, the inflammation can usually subside within 24 hours, and the pain can be relieved. If the effect is not significant, then the incision and drainage method can be adopted again.

There are two treatments for Bartholin's gland cysts. One is cystectomy, which is to remove the cyst completely. However, due to the large trauma, it is now rarely used. Instead, cystostomy is used. The incision of the ostomy is chosen below the cyst to allow the cyst fluid to flow out. At the same time, a drainage strip is used to prevent adhesion of the stoma. A 1/5000 potassium permanganate solution is used for sitz bath. The prognosis is generally good, and the function of the Bartholin's gland can also be well preserved.
