When suffering from trichomonas vaginitis or bacterial vaginitis, or cervical erosion, the leucorrhea increases significantly, and the inflammatory secretions irritate the vulvar skin and mucous membranes, causing itching. In addition, pubic lice, genital scabies and some sexually transmitted diseases can also cause vulvar itching. Pinworm disease is more common in young girls, mainly causing itching around the anus, which can spread to the vulva and occur at night.

Clinical symptoms of vulvar pruritus

Typical symptoms: leucorrhea, anal fissure, anal itching, eczema, papules, and dizziness.

1. Vulvar pruritus

Vulvar pruritus mostly occurs in women before and after menopause, and may be related to endocrine disorders, low sex hormone levels and menopausal autonomic dysfunction. Patients are often accompanied by symptoms such as hyperhidrosis, emotional instability and insomnia. The itching areas are mainly in the labia majora and labia minora, but the mons pubis, clitoris and vaginal mucosa are also often itchy. In severe cases, it may involve the area around the anus and perineum. Drinking, mood changes, high temperature of bedding, scratching and friction, and even certain cues may induce the onset or aggravation of the itching. The degree of itching varies from person to person and can be mild or severe, and the duration is also different. The itching is paroxysmal and severe at night. Severe itching can be unbearable, making the patient restless.

Because severe itching affects sleep, symptoms of neurasthenia such as dizziness, depression and loss of appetite may occur. Due to constant scratching, secondary damages such as strip scratches, blood scabs, papules, eczematous changes, lichenoid changes and pigmentation may appear. The skin of the labia is often infiltrated and thickened, and the clitoris and vaginal mucosa may become red, swollen and eroded. Local ulcers due to scratching may cause secondary infection.

Pruritus caused by different causes may have different clinical manifestations. Vulvar pruritus caused by candidal vulvovaginitis may show white tofu-like leucorrhea, and Candida growth may be found in microscopic examination or culture of the secretions. Vulvar pruritus caused by trichomoniasis has more leucorrhea in a foamy state, and vaginal Trichomonas may be found. Pruritus caused by gonococci generally has more purulent leucorrhea. Atrophy of the vulvar skin in elderly women can also cause vulvar pruritus. Vulvar pruritus caused by contact with contraceptives, rubber menstrual belts, diabetes, etc. is also not uncommon. However, there are also some stubborn vulvar pruritus (including unmarried young women), and the current examination methods cannot find the cause.

2. Anal itching

It is usually more common in middle-aged men, but women can also get it, and children with pinworm disease can also suffer from it. The itching is usually limited to the anus and the skin around it, and can also spread to the skin of the perineum, vulva or scrotum. Anal itching caused by pinworm infection is more common in children, and often worsens at night, and pinworms or eggs can be found (the positive rate is high at night). Anal itching in adults is often caused by hemorrhoids, anal fissures, etc., and many patients cannot find the cause, and the symptoms of anal itching are very serious. Due to frequent scratching, the anal folds may show secondary damage such as hypertrophy, radial fissures, infiltration, lichenification or eczematous changes.
