How to choose hormone therapy for patients with dysmenorrhea

Dysmenorrhea can be diagnosed if the pain occurs during menstruation and disappears after menstruation, and the pelvic examination is normal. However, attention should be paid to secondary dysmenorrhea caused by organic lesions. Sometimes the two types of dysmenorrhea occur at the same time. Dysmenorrhea refers to lower abdominal pain that coincides with the menstrual cycle. The incidence rate is about 90%. About 50% to 60% of patients need to use analgesics, and only 10% of them affect their work and life.

Patients with dysmenorrhea can choose hormone treatment, such as oral contraceptives, progestins, artificial cycle treatment, antispasmodics, morphine analgesics, etc.

1. Oral contraceptives: The usage is the same as contraception. Short-acting contraceptives are taken from the 3rd to 5th day of menstruation to relieve dysmenorrhea by inhibiting ovulation. Due to the decrease in estrogen secretion by the ovaries, the endometrium is not proliferating properly and the amount of menstrual blood is reduced, which is another reason for relieving dysmenorrhea. Generally, it is taken for 3 to 6 cycles in a row. Since the function of the hypothalamus-pituitary-ovarian axis in adolescent girls is unstable, contraceptives may inhibit its function and cause amenorrhea after stopping the drug, so it is not the first choice.

2. Progesterone: Supplementing progesterone 5 to 7 days before menstruation can relieve some dysmenorrhea

3. Artificial cycle treatment: used for those with uterine hypoplasia.

4. Antispasmodics: Atropine 0.5 mg subcutaneously can relax the isthmus of the uterus and relieve dysmenorrhea

5. Morphine analgesics: Because morphine analgesics can be addictive, they should be used with caution.

6. Chinese patent medicine: Chinese patent medicines for the treatment of dysmenorrhea include Tongjing Pills, Jiawei Xiaoyao Powder, etc. You can also take Chinese medicine based on syndrome differentiation and treatment.
