2. Primary dysmenorrhea: the main cause of dysmenorrhea.

(1) Autologous hormone factors: Primary dysmenorrhea often occurs during the ovulatory menstrual cycle, indicating that primary dysmenorrhea is related to the release of autologous hormones after ovulation. The content of PGs in the endometrium also changes periodically. The concentration of PGFa gradually increases from the endometrial proliferation phase to the secretory phase, reaching a peak during the menstrual period. Adolescent girls do not ovulate or have dysmenorrhea, and the PCs content at that time is only 1/5 of that in the ovulatory cycle; after taking oral contraceptives, the amount of PGs in menstrual blood drops below normal levels. These phenomena indicate that ovarian hormone imbalance can affect the synthesis of D in the endometrium and cause dysmenorrhea, which is a very common cause of dysmenorrhea.

(2) Prostaglandin factors: Studies have found that patients with dysmenorrhea have increased uterine cavity pressure, increased contraction frequency, uncoordinated contractions, and disordered rhythms, which lead to decreased uterine blood flow and hypoxia, causing severe pain in patients. When patients were given an intravenous injection of 250 ug of the B2 receptor agonist mesalbutamol, uterine contractions disappeared, local blood flow improved significantly, and pain was completely relieved. This shows that primary dysmenorrhea is caused by excessive uterine contractions that cause myometrial ischemia.

(3) Vasopressin factors: It is known that vasopressin containing arginine can cause strong contraction of the uterus of non-pregnant women, and the local blood flow decreases significantly. The plasma concentration of vasopressin and the responsiveness of the uterus to it vary with the menstrual cycle. It is most sensitive at the beginning of menstruation. The plasma AVP concentration of patients with primary dysmenorrhea on the first day of the menstrual cycle is significantly increased, which is 1-4 times that of the control group. In addition, the use of desaminoethyl oxytocin to competitively inhibit the effect of AVP on the uterus can significantly relieve the symptoms of primary dysmenorrhea. Therefore, AVP may also be an important factor in primary dysmenorrhea.

(4) Psychological factors: Pain often occurs in women with immature psychological development and neurotic personalities. Some girls do not have a good understanding of menstrual physiology and feel fear and anxiety when their period is approaching. Adverse stimuli such as stressful study or work, sudden changes in the environment, and unhappy moods can all cause dysmenorrhea through the hypothalamus-pituitary-ovarian axis, leading to changes in hormones in the body. However, studies in recent years have shown that it is difficult to attribute dysmenorrhea to psychological factors. Psychological factors can have a certain impact on acute and chronic pain in any part of the body, and their effect on primary dysmenorrhea is not more unique, so this factor has not been taken seriously.
