1. Too large: The ovarian cyst is too large, and the capsular nerves are stimulated, causing reflex pain, so it hurts when pressed or you feel pain. Surgical removal of the cyst can be a treatment option.

2. Secondary infection: Ovarian cysts may cause secondary infection. Due to inflammatory stimulation, the cysts may cause pain when pressed. Anti-infection treatments such as cefaclor, doxycycline, and amoxicillin can be given, and cystectomy can be performed after the inflammation is controlled.

3. Ovarian cyst pedicle torsion: When the ovarian suspensory ligament and related soft tissue are twisted, venous return is blocked and the tumor is severely congested, which may manifest as pressure pain, nausea, vomiting and other symptoms. After diagnosis, the affected side can be treated by oophorectomy.

4. Rupture of ovarian cyst: Acute torsion of ovarian cyst causes obstruction of venous return and rapid increase of fluid. Blood vessels rupture and bleed, causing pain when pressing. Surgical resection is required as soon as possible to avoid abdominal infection.
