Treatment of pregnancy complicated with uterine fibroids

The incidence of uterine fibroids combined with pregnancy accounts for about 0.5% to 1% of fibroid patients, which will have a certain impact on women's pregnancy and childbirth. Because physiological changes occur in various systems and organs of women during pregnancy, the treatment of pregnancy combined with uterine fibroids should be paid more attention. The treatment principles include the following:

1. Treatment of asymptomatic patients

Generally speaking, those who are asymptomatic during pregnancy do not need special treatment, but they should undergo regular checkups and closely monitor the condition of the fibroids.

2. Prevent bleeding

Most patients with pregnancy complicated by uterine fibroids can give birth naturally, but postpartum hemorrhage should be prevented. If a patient with postpartum hemorrhage has shock, the shock should be corrected in time, the amount of bleeding should be recorded, and supportive therapy should be given; during the observation period or after surgery, attention should be paid to improving anemia, and blood routine tests should be performed regularly; in case of small or moderate vaginal bleeding, broad-spectrum antibacterial drugs and uterine contractants should be given.

3. Perform cesarean section when necessary

If the fibroids hinder the descent of the fetus, a cesarean section should be performed. Whether to remove the fibroids during the operation depends on the size and location of the fibroids and the patient's condition. Generally speaking, fibroids during pregnancy are rich in blood vessels and have unclear boundaries. They are prone to cause heavy bleeding when peeled off. It is not appropriate to remove the fibroids during cesarean section. However, subserosal fibroids or pedunculated fibroids can be removed during cesarean section.
