2. Hyperplastic dystrophy

It usually occurs in women in their 30s and 60s. The main symptoms are unbearable itching of the vulva accompanied by local pain after scratching. The range of lesions varies, mainly affecting the labia majora, the groove between the labia, the clitoral hood and the posterior symphysis, and is often symmetrical. The skin in the affected area is thickened like leather, with raised wrinkles, or scaly and eczematous changes. 3. Hyperplastic or mixed type with atypical epithelial hyperplasia

It is generally believed that among proliferative and mixed lesions, only 5 to 10 cases may have atypical hyperplasia, and this atypical hyperplasia is limited to the proliferative epithelial cell part. Atypical hyperplasia often has no special clinical manifestations, and local tissue biopsy is the only diagnostic method. However, if ulcers or clear-cut white bulges appear locally on the vulva, if atypical hyperplasia is found in the biopsy, the possibility of cancer is high.

4. Lichen sclerosus dystrophy

It can occur at any age, including young girls under 10 years old, but it is more common in women around 40 years old. The main symptoms of white vulvar lesions are itching in the lesion area, but it is generally much milder than the proliferative lesions. In the late stage, sexual intercourse difficulties occur. The lesions involve the skin, mucous membranes and perianal skin of the vulva. The clitoral foreskin, labia minora, and posterior commissure are common lesion sites. In addition to the skin or mucous membrane turning white and thin, drying and cracking, and losing elasticity, the clitoris is often atrophied and adhered to the foreskin, and the labia minora is flat and disappears. In the late stage, the skin is thin and wrinkled like cigarette paper, and the vaginal opening is contracted and narrow, only the fingertips can fit. Young girls with this disease often feel discomfort in the vulva and perianal area after urination or defecation. Examination reveals keyhole-shaped pearl yellow mottled or white lesions in the vulva and perianal area. But generally by puberty, the lesions can disappear on their own. Some postmenopausal women have thin and shiny vulvar skin, mild tingling, itching and dyspareunia, which was previously called "primary vulvar atrophy". However, except for the absence of hyperkeratosis of the epidermis, other tissue changes are the same as those of lichen sclerosus.
