The main differential diagnosis of acute pelvic inflammatory disease includes: acute appendicitis, ectopic pregnancy, ovarian cyst pedicle torsion, etc.; the main differential diagnosis of chronic pelvic inflammatory disease includes: endometriosis and ovarian cancer. 3. Systemic symptoms of chronic pelvic inflammatory disease include low fever and fatigue. Some patients may experience neurasthenia due to the long course of the disease, such as insomnia, depression, and general discomfort. Lower abdominal swelling, pain, and lumbar pain usually worsen after fatigue, sexual intercourse, and before and after menstruation. Due to chronic inflammation, pelvic congestion, menorrhagia, ovarian damage, and fallopian tube adhesions and obstruction can lead to infertility. 4. Symptoms: The patient has an acute appearance, fever, accelerated heart rate, lower abdominal muscle tension, tenderness, and rebound pain. Gynecological examination shows that there is a large amount of purulent secretion flowing out of the cervix, the circle is obviously painful, and the posterior circle may be full and fluctuating, indicating pelvic abscess; cervical congestion and obvious pain; uterine tenderness, limited activity; obvious tenderness on both sides of the uterus, thickening of the fallopian tubes can be touched, and there is obvious tenderness; if it is an abscess, a tumor with obvious tenderness can be touched, with a sense of fluctuation; in case of parauterine connective tissue inflammation, a flake increase on one or both sides of the parauterine can be touched, or both sides of the uterine fundus ligament are highly edematous and thickened, with obvious tenderness. |
