Obstetrician and gynecologist Cai Fengbo said that when the BMI is less than 17, the probability of infertility is 1.6 times that of normal women. According to a study published in The New England Journal of Medicine (NEJM), the risk of death for subjects with a BMI ≤ 15 increased by approximately 2.0 to 2.8 times.

Being as thin as a "paper man" has serious health hazards. Dr. Cai Fengbo shared a case he treated in clinical practice. A 25-year-old unmarried lady was so thin that she was just skin and bones when she came to see the doctor. The patient said that she had not had her period for three years and asked the doctor to help induce her period. After diagnosis, it was found that the patient, who was 157 cm tall, lost 13 kg in one year from 42 kg (BMI was 17) to 29 kg (BMI was 11.7) due to deliberate weight loss. So she was advised to have a blood test to find out if the problem was with the pituitary gland or if the amenorrhea was simply caused by weight loss. Because of changes in pituitary gland function, menstruation may also be delayed for a long time!

What was even more shocking was that she also had the habit of picking her throat to lose weight, and there were scars on her throat caused by this. What is throat-clenching weight loss? Many girls want to lose weight but they cannot control their mouths. Every time after eating, they will put their hands into their throats to induce vomiting and spit out what they have eaten. Picking the throat to lose weight is a weight loss method that many people try, but this extreme weight loss method has hidden dangers.

Anorexia nervosa may be life-threatening

Later, the patient's blood test report showed that her excessive weight loss had caused hormonal chaos, so her menstruation had not come for a long time. Therefore, she was strongly advised not to lose weight anymore, as further weight loss would seriously affect her chances of having children in the future. She might even develop anorexia, which could be life-threatening! And tell the patient to gain weight to a BMI of 18.5 or above (45.6 kg), and then menstruation should become regular naturally. And in order to induce menstruation, hormones are first prescribed to make the menstruation come every month.

When a woman is underweight, the pituitary gland will secrete insufficient follicle-stimulating hormone (FSH) and luteinizing hormone (LH), leading to reduced ovarian secretion and gradually causing chronic anovulation and infertility. Body mass index (BMI) is calculated by dividing your weight in kilograms by your height in meters squared.

BMI<18.7→underweight

BMI=18.7~24.99→normal weight

BMI=25~29.99→overweight

BMI ≧ 30 → Obesity

The ideal BMI for women is 20.8. Women with a BMI less than 17 have a 1.6 times higher chance of infertility than normal women.

According to the Department of Epidemiology at the Harvard School of Public Health, 597 women diagnosed with ovulatory infertility in seven infertility clinics in the United States and Canada were selected as subjects, including 1,695 primiparas. The study found that, with a normal BMI defined as 20-24.9, obese women (BMI > or = 27) had a relative risk of ovulatory infertility of 3.1 compared with women with a lower body weight. For women with a BMI of 25-26.9 or below 17, the relative risk (RR) of infertility is = 1.6, which is 1.6 times higher than that of the average woman. The risk of ovulatory infertility was highest in obese women, but was also slightly increased in moderately overweight and underweight women.

In addition, the study published in The New England Journal of Medicine (NEJM) was mainly based on Asian countries, with the participation of Academia Sinica in Taiwan. A total of 1,155,676 subjects were included. The results showed that people with a BMI of more than 35 had a 1.5-fold risk of death; in the group with a BMI below the reference range of 22.6 to 25.0, the risk of death was increased, and the risk of death for subjects with a BMI ≤ 15 increased by approximately 2.0 to 2.8 times.

The ideal BMI value for women is 20.8, which is between 18.7 (lower limit) and 23.8 (upper limit). If it exceeds the upper limit, it is considered overweight; the ideal BMI value for men is 22, which is between 20 (lower limit) and 25 (upper limit). If it exceeds the upper limit, it is considered overweight.

Excessive weight loss is harmful to health, be careful of anorexia nervosa

Dr. Cai Fengbo reminds us that obesity has become a major health problem in this century and the next. In the United States, one in three women asks a doctor for help in losing weight, and domestic slimming centers have a large number of consultants. In fact, excessive weight loss will interfere with the normal functioning of the endocrine system, interfere with ovulation, hinder vitality and cause infertility, and even lead to anorexia nervosa. If you lose weight to the point of developing anorexia nervosa, it can seriously lead to organ failure and an increased mortality rate, so you must be careful!
