The most common cause of vulvar itching is nonspecific vulvitis, which is an inflammation of the vulvar skin or mucous membrane caused by physical, chemical and other non-pathogen factors. The vulva is easily irritated by menstrual blood and vaginal secretions. If the patient does not pay attention to cleanliness, or if patients with fecal leakage are irritated by fecal contamination, or patients with urinary fistula are soaked in urine for a long time, etc., nonspecific inflammatory reactions can be caused. Physical and chemical irritation caused by wearing tight synthetic underwear for a long time or using sanitary products for a long time during menstruation, such as friction between skin and mucous membranes, local moisture, poor air permeability, etc., can also cause nonspecific vulvitis.

The vulvar skin and mucous membranes are itchy, painful, and burning, which worsens during activities, sexual intercourse, urination, and defecation. In the acute inflammation stage, the vulva is found to be congested, swollen, and eroded, often with scratches, and in severe cases, ulcers or eczema form; in chronic inflammation, the vulvar skin is found to be thickened, rough, chapped, and even lichenified.

The principle of treatment is to eliminate the cause, keep the vulva clean and dry, and provide symptomatic treatment.

1. Etiological treatment: Find and actively eliminate the cause and improve local hygiene. If diabetes is found, it should be treated promptly. If there is urinary fistula or fecal fistula, it should be repaired promptly.

2. Local treatment Keep the vulva clean and dry, and clean the vulva in time after defecation. You can use 0.1% povidone iodine solution or 1:5000 potassium permanganate solution for sitz bath, twice a day, 15-30 minutes each time. Apply antibiotic ointment or Chinese patent medicine ointment after sitz bath. You can also use Chinese medicine decoction to fumigate and wash the vulva, 1-2 times a day.
