In severe cases, the entire pelvic peritoneum undergoes inflammatory changes, and in a few cases, it can even spread to the entire abdomen, becoming diffuse peritonitis. Sometimes, due to pelvic peritonitis or inflammatory lesions of other abdominal organs, pus accumulates in the lowest part of the abdominal cavity, the uterine rectal fossa, forming a uterine rectal fossa abscess. Pelvic peritonitis is not a transmitted disease and is not contagious, but it is also very harmful to the female body and must be treated in time. [Diagnosis] It is not difficult to diagnose based on the medical history, symptoms and signs, but care should be taken to distinguish it from acute appendicitis perforation. In addition, some diseases have symptoms similar to acute pelvic peritonitis, such as torsion of the pedicle of the appendix tumor, rupture of the fallopian tube hematoma, necrosis of uterine fibroids, etc., especially the peritoneal irritation symptoms caused by massive abdominal bleeding (such as rupture of fallopian tube pregnancy), which are sometimes difficult to distinguish from the local signs of pelvic peritonitis and diffuse peritonitis. In a few cases, pelvic peritonitis extends to the gallbladder, causing right upper abdominal pain and tenderness, which is easily confused with the signs of cholecystitis. In such cases, the medical history should be carefully inquired, and other systemic conditions (such as body temperature, blood pressure and general condition) should be referred to before a clear diagnosis can be made.

Chronic pelvic peritonitis is often accompanied by chronic salpingo-oophoritis and chronic uterine myositis, so it often has all the symptoms of chronic pelvic genital inflammation. The treatment principles are exactly the same, so there is no need to identify them one by one.

[Treatment measures] Similar to the treatment of acute salpingo-oophoritis, pelvic peritonitis has its own protective function, so surgery should not be performed indiscriminately, which will damage and spread the inflammation. Therefore, its treatment principles are different from those of diffuse peritonitis. Non-surgical treatments such as systemic supportive therapy and infection control should be selected. However, if an abscess is formed, it should be drained through the abdomen or vagina.
