Tuberculous endometrial tuberculosis is caused by infection with Mycobacterium tuberculosis and is often secondary to fallopian tube tuberculosis.

Non-tuberculous endometrial tuberculosis is more common and is caused by infection with general pathogens. It is common during menstruation, abortion and after delivery: such as retained placenta and fetal membranes during delivery, sexual intercourse during menstruation, prolonged uterine bleeding, incomplete abortion infection, gynecological examinations without strict disinfection, ascending infection from intrauterine operations such as artificial abortion and various vaginal surgeries, ascending infection from cervical tuberculosis, ascending infection from vaginal tuberculosis, and infection caused by endometrial polyps or submucosal myoma necrosis.

Endometrial tuberculosis is caused by bacteria ascending along the vagina and cervix or descending along the fallopian tubes and reaching the endometrium through the lymphatic system. Most cases are caused by ascending from the vagina and cervix.
