Treatment of Bartholinitis in Traditional Chinese Medicine

When the Bartholin's glands are acutely inflamed, the vulva will be red, swollen, painful on one or both sides, and even become abscesses with pus. When the inflammation improves, the cyst fluid is left behind to become a Bartholin's gland cyst, but acute attacks may recur. This is a category of "vulvar swelling" and "vulvar sores" in traditional Chinese medicine.

【Etiology and pathology】

This disease is caused by dampness and heat infiltrating the vulva, which may lead to toxicity and suppuration. It can also be caused by cold stagnation of qi and blood, which may cause stasis in the skin.

1. After the heat and toxic period, if hygiene is neglected or the vulva is damaged, it may be infected with evil toxins or damp-heat toxins, which may lie in the liver meridian. The damp-heat toxins will flow into the genitals along the meridians, fight with the blood and qi, and form blisters.

2. Cold stagnation: Cold evil stagnates qi and blood, stagnates inside, and evil qi cannot go out, sinking into the skin. Or, if the yin is usually deficient, qi and blood are blocked and condensed into lumps with dampness.

【Diagnosis points】

1. Based on the symptoms of redness, swelling, heat and pain on the vulva, or a lump with yellow water dripping, the quality is relatively large. When an abscess is formed, a sense of fluctuation can be felt, accompanied by systemic symptoms such as fever. When the pressure inside the abscess increases, the abscess may rupture on its own, and the disease can be diagnosed.

2. The acute phase of this disease must be differentiated from vulvar eczema and acute vulvitis.

3. When Bartholin's gland cyst occurs in the chronic stage of this disease, it must be differentiated from lipoma and vulvar sebaceous cyst.

【Dialectical classification】

1. Heat toxicity: One or both sides of the vulva are red, swollen and painful, like a cocoon. It is not easy to subside, and it is difficult to move and sit or lie still. After a few days, it may become pus and burst, and the pus is foul-smelling and thick. It may heal in about a week. There is often repeated pus discharge and the formation of sinus tracts. Accompanied by aversion to cold and fever, dry mouth and poor appetite, painful urination, restlessness and poor sleep, red tongue, yellow and greasy fur, and stringy and rapid pulse.

2. Cold stagnation: swelling of one or both sides of the vulva like a cocoon, with unchanged skin color, heaviness and discomfort, which may cause difficulty in walking. It often increases before and during menstruation and returns to its original state after menstruation, accompanied by pale complexion, poor appetite, loose stools, fatigue, pale tongue with thin coating, and weak pulse.

【Treatment by type】

1. Heat and toxicity treatment: clear away heat, detoxify, promote blood circulation and remove blood stasis.

Prescription: Wuwei Xiaodu Drink with added ingredients.

Dandelion 15g, honeysuckle 9g, wild chrysanthemum 6g, Viola yedoensis 15g, Achyranthes bidentata 9g, frankincense 6g, Moutan bark 12g, red peony root 12g, Gleditsia sinensis 12g. Add or subtract: For those with fever and chills, add Schizonepeta tenuifolia 9g, Saposhnikovia divaricata 9g, and Mentha 6g (added later); for those with painful urination, add Polygonum multiflorum 10g and Polygonum multiflorum 10g; for those with pus, add Coix lachryma-jobi 15g; for those with severe heat toxicity, add Coptis chinensis 6g, Scutellaria baicalensis 10g, and Phellodendron chinense 10g.

2. Treatment for cold stagnation: tonify Qi and nourish blood, raise Yang and reduce swelling.

Prescription: Buzhong Yiqi Decoction with modifications.

15g of Astragalus, 12g of Angelica, 9g of Cimicifuga, 9g of Bupleurum, 6g of Tangerine Peel, 12g of Atractylodes, 15g of Bombyx Batryticatus, 4.5g of Gleditsia sinensis, 12g of Cassia Twig, 6g of Roasted Licorice. Add or subtract: If the patient has loose stools and poor appetite, add 15g each of raw and cooked Coix seeds and 6g of chicken's gizzard lining.

【Chinese patent medicine】

1. Take Sanhuang Tablets 3 times a day, 5 tablets each time, and swallow. Suitable for heat toxicity type.

2. Take Yiqing Capsules 3 times a day, 3 capsules each time, and swallow. Suitable for heat toxicity type.

3. Take Buzhong Yiqier twice a day, 6 grams each time, swallow. Suitable for cold stagnation type.

【Simple recipe】

1. External application method: Apply golden ointment or jade dew ointment to the local redness and swelling.

2. Fumigation and washing method (1) Tayang Decoction: 30g of Heshi, 15g of Sophora flavescens, 15g of Clematidis, 15g of Radix Angelicae Sinensis, 15g of Cnidium monnieri, 15g of Wolfbane Chamaejasme, decocted in water for fumigation and washing.

(2) Prescription: 15 grams of honeysuckle, 30 grams of dandelion, 10 grams of red peony root, and 30 grams of wild yam, decocted in water for fumigation and washing.

(3) Intracystic injection: For Bartholin's gland cyst, anhydrous ethanol can be used

【Notes】

1. After menstruation and childbirth, you should pay attention to personal hygiene, keep the vulva clean, use personal bathing utensils, and change underwear every day to prevent external infection.

2. In case of acute infection, apply local hot or cold compress and give antibacterial treatment. If an abscess is formed, it should be incised promptly and the drainage thread must be unobstructed.

3. If the Bartholin's gland cyst is small and asymptomatic, no treatment is required. However, if it is large or recurrent, a Bartholin's gland cyst ostomy should be performed to facilitate the discharge of secretions, or the cyst should be removed.
