How to check for pituitary amenorrhea

There are many reasons for female amenorrhea, one of which is pituitary factors. Some relevant examinations can help detect lesions early and deal with them in time.

1. Vaginal exfoliated cell examination

Guided cell examination is a commonly used method to understand estrogen levels. After soaking a cotton swab in saline, remove the exfoliated cells from the upper side wall of the vagina, apply them on a glass slide, fix and stain, and observe the percentage of cells in the surface, middle and bottom layers. The higher the percentage of surface cells, the higher the estrogen level.

2. Cervical mucus examination

Examining the properties of cervical mucus can help diagnose pituitary amenorrhea. If the cervical mucus of an amenorrhea patient is transparent, thin mucus with good tensile strength, and fern-like crystals can be seen under a microscope after being applied on a glass slide and dried, it indicates that the patient's ovaries have the function of secreting estrogen.

3. Estrogen test

For patients with amenorrhea who have a negative progesterone test, take oral diethylstilbestrol 1 mg/d, ethinyl estradiol 10 μg/d, or other estrogens with equivalent biological effects for 20 consecutive days. Add progesterone 20 mg/d by intramuscular injection for the last 3 to 5 days. Observe for withdrawal bleeding 3 to 7 days after stopping the drug. If withdrawal bleeding occurs, it means that the endometrium responds to the effects of estrogen and progesterone, and can undergo normal growth and shedding changes. The cause of amenorrhea should be in the ovary or higher.
