Lower abdominal pain is a typical symptom of adnexitis. During the diagnosis process, it is necessary to differentiate the patient based on the duration of the lower abdominal pain and the severity of the onset.

1. Identify according to the time of lower abdominal pain

Lower abdominal pain in patients with adnexitis is usually more obvious when tired or before and after menstruation, and is unilateral pain, accompanied by varying degrees of increased leucorrhea, back pain, and irregular menstruation. Short-term transient lower abdominal pain in the middle of menstruation may be ovulation pain, or caused by primary dysmenorrhea or endometriosis; if a woman experiences lower abdominal pain after abortion, it should be considered whether there are symptoms of postoperative cervical or uterine adhesions. It should be noted that if adolescent girls experience periodic lower abdominal pain and are not menstruating, they should go to the hospital in time to check for congenital genital malformations, such as hymen occlusion.

2. Degree of pain in the lower abdomen

Lower abdominal pain caused by adnexitis can be divided into acute and chronic types. Acute pain is more severe and sudden, while chronic pain usually starts slowly and gradually worsens, accompanied by a feeling of heaviness and pulling in the lower abdomen, which can be severe or mild at times, and the entire appendage tends to thicken. Lower abdominal pain caused by malignant tumors usually occurs suddenly, especially when there is ovarian pedicle torsion, ovarian cyst rupture, or ectopic pregnancy, and needs to be carefully identified.
