The clinical diagnosis of chronic adnexitis needs to be differentiated from old ectopic pregnancy. The diagnostic process first starts with symptoms and signs to find out the characteristics of each disease.

1. Chronic adnexitis

Chronic adnexitis patients have chronic pain in the lower abdomen as the main symptom, and the pain is most obvious 2 cm above the midpoint of the inguinal ligament. In severe cases, there may even be refusal to press, abdominal muscle rigidity, and rebound pain. The abdominal pain will be aggravated during defecation, and sometimes there will be symptoms such as urinary pain, constipation, abdominal distension, and mucus in the stool. On this basis, patients are often accompanied by increased leucorrhea, lumbar pain, menstrual disorders, etc., and the symptoms are significantly aggravated during menstruation or after fatigue. In addition, patients with chronic adnexitis generally have facial flushing symptoms, and most of them will also have bilateral tender masses during examination.

2. Chronic ectopic pregnancy

Most patients with old ectopic pregnancy are accompanied by short-term delay of menstruation, and the patients may experience sudden lower abdominal pain, nausea, dizziness, and even syncope, which may be relieved after a period of time, and even return to normal life. Later, they may have repeated sudden abdominal pain symptoms, which is different from chronic adnexitis. In addition, patients with old ectopic pregnancy often have anemia, and the abdominal mass is mostly on one side, which is a substantial and elastic mass, which can also be distinguished from chronic adnexitis.
