1. Weak Qi and blood

Main symptoms: dull pain in the lower abdomen during or after menstruation, which is pleasing to massage, light-colored and light-volume menstruation, thin and watery menstruation, accompanied by fatigue, pale complexion, pale tongue with thin coating, and weak and thready pulse. This is a common classification of dysmenorrhea.

Analysis: Physical weakness leads to insufficient Qi and blood. After menstruation, the blood sea is empty, the uterus vessels are not nourished, or the body is weak and the Yang Qi is not strong, and the blood cannot be transported, so there is dull pain in the lower abdomen during menstruation or after menstruation, and the patient likes to massage; Qi deficiency leads to insufficient Yang Qi, and blood deficiency leads to insufficient essence and blood, so the menstrual blood is light in color, small in quantity, and thin in quality. The remaining symptoms are also signs of blood deficiency and weak Qi.

2. Liver and kidney deficiency

Main symptoms include dull pain in the lower abdomen and soreness in the waist after menstruation, small amount of thin menstrual blood, dark menstrual color, or dizziness and tinnitus, empty and cold lower abdomen, pale tongue, thin white fur, and deep and fine pulse.

Analysis: This type of dysmenorrhea is characterized by liver and kidney deficiency, Chong and Ren deficiency, insufficient essence and blood. After menstruation, the blood sea becomes even more deficient, and the uterine vessels are malnourished, so there is dull pain in the lower abdomen after the menstruation; deficiency of essence and blood, damage to yin and yang, scanty menstruation with light color and thin texture, and an empty and cold lower abdomen; deficiency of kidney and essence, malnourished clear orifices, so there is dizziness and tinnitus; the waist is the home of the kidneys, and kidney deficiency causes back pain.

3. Dampness and heat

Main symptoms: abdominal distension and pain before and during menstruation, heavy menstrual flow, red color, thick or clotted, yellow or foul-smelling vaginal discharge, red tongue with yellow greasy coating, and stringy and rapid pulse. This type of dysmenorrhea is relatively common.

Analysis: Exogenous or internal dampness and heat flow into the Chong and Ren meridians, blocking the Qi and blood, causing menstruation to be sluggish, resulting in abdominal pain during menstruation; heat disturbing the Chong and Ren meridians will result in excessive menstrual flow with red and clots; heat burning body fluids will result in thick menstrual fluid; dampness and heat flowing down and damaging the Ren and Dai meridians will result in yellow or foul-smelling vaginal discharge. The tongue and pulse are signs of internal dampness and heat.
