Uterine amenorrhea and its causes

Uterine amenorrhea is caused by the pathology or loss of function of the uterus itself. The endometrium is the source of menstrual blood. Even if the hypothalamus-pituitary-ovarian function is normal, the uterus itself cannot respond to sex hormones (estrogen and progesterone) and cannot undergo cyclical changes, and there will be no menstruation. Common causes of uterine amenorrhea are:

(1) Congenital uterine dysplasia

①Primary amenorrhea.

② Gynecological examination may reveal various manifestations such as absence of uterus, incomplete uterine development, solid uterus, and absence of endometrium.

③Uterine cavity exploration, diagnostic curettage, B-ultrasound laparoscopy, chromosome karyotype analysis to identify sex chromosomes, etc. can assist in the diagnosis of the cause.

(2) Hysterectomy or endometrial destruction

① The patient has a history of physical uterine trauma, such as curettage, intrauterine radiation therapy, or a history of severe infection, such as severe infection after delivery or abortion, endometrial tuberculosis, or has undergone hysterectomy.

②Estrogen and progesterone induced menstruation test was negative.

③ Exploration of the uterine cavity may reveal dark red blood flowing out.

④Diagnostic curettage and hysteroscopy can assist in diagnosis.
