What to do if endometrium is thick?

Normal endometrium of childbearing age is 8-12mm. The thickness of endometrium is generally considered to be the following diseases:

1. Endometrial lesions include simple hyperplasia, complex hyperplasia, atypical hyperplasia and malignant endometrial lesions;

2. Endometrial polyps;

3. Due to long-term anovulation and lack of progesterone conversion, the normal endometrium thickens;

4. Local endometrial problems;

For patients with thickened endometrium, the focus is on ovulation.

If ovulation occurs

1. Perform vaginal ultrasound after menstruation to understand whether the thickness of the endometrium is uniform. If the endometrium is still thick at this time, a hysteroscopy may be performed if necessary:

① If the patient has no clinical symptoms and the endometrial polyp is smaller than 2 cm, it can be observed;

② The endometrium shows polyp-like changes, the patient ovulates, and menstruation is normal, and can be observed for a period of time.

2. If there is no ovulation

The absence of ovulation indicates the absence of progesterone. Under the action of estrogen alone, the endometrium has long been characterized by endometrial proliferation or hyperplasia, including simple hyperplasia, complex hyperplasia, and atypical hyperplasia. Lack of supplementation requires regular progesterone supplementation to fully transform the endometrium. For complex endometrial hyperplasia and atypical hyperplasia, large doses of progesterone are required for treatment.
