TCM Syndrome Differentiation and Treatment of Irregular Menstruation in Adolescence

Traditional Chinese medicine believes that irregular menstruation in adolescent girls is mostly due to insufficient kidney qi or imbalance of yin and yang. The classics say: "When a girl is seven years old, her kidney qi is strong, her teeth change and her hair grows. At the age of 27, the menstrual period arrives, the Ren meridian is open, the Taichong meridian is strong, and the menstruation comes on time, so she can have children."

The kidney is the foundation of the innate constitution. It stores essence and qi, controls bones and produces marrow. The marrow is connected to the brain and is the foundation of human growth, development and reproduction. The brain is the sea of ​​marrow. If the sea of ​​marrow is empty, insufficient kidney essence may lead to reproductive dysfunction or decline, resulting in Chong and Ren disorders and menstrual disorders. Menstrual disorders caused by kidney deficiency are mostly caused by weak kidney qi and Chong and Ren disorders. The growth and development of the human body depends on the balance of yin and yang in the kidney, which cannot be too strong or too weak. Doctors say: "Menstruation is a regular observation, which means observing the evolution of yin and yang in the whole body. To know its safety and danger, it comes once a month. Too much or too little is imbalance. If yin is too much, it will come early, and if yin is insufficient, it will come late. Sometimes it is more, sometimes it is less. It can't stop, and it can't stop. It's all caused by the rise and fall of yin and yang."

Those with insufficient kidney essence and qi often have their menarche delayed, with light color and regular amount of menstruation, and they feel sore, weak and powerless in the waist. The treatment should focus on tonifying the kidney first, and Jinkui Shenqi Pills should be taken as a decoction. Those with yin deficiency often have their menstruation early, with heavy red amount of menstruation, fever in the five hearts, red tongue and thready pulse. The treatment should focus on nourishing yin and blood first, and Zuoguiyin or Liangdi Decoction combined with Erzhi Pills should be used. Those with heavy and continuous bleeding should use Siwu Decoction with blood-nourishing, qi-invigorating and bleeding-stopping ingredients. Commonly used prescriptions include Siwu Decoction with modifications as described in Fu Qingzhu's Gynecology, which is good for strengthening the kidney and stopping bleeding.
