Because the causes of dysmenorrhea are relatively complex, the diagnosis of dysmenorrhea often requires more caution. Traditional Chinese medicine has a long history in the diagnosis of dysmenorrhea and has accumulated a lot of clinical experience. Here we will briefly introduce the diagnostic criteria of dysmenorrhea in traditional Chinese medicine. 1. Cold and dampness stagnation type The diagnostic basis for dysmenorrhea of cold-dampness stagnation type is mainly cold pain in the lower abdomen during menstruation, pain relieved by heat, severe pain when pressed, scanty menstrual flow, dark menstrual blood with clots, or fear of cold and body pain, white greasy tongue coating, and deep and tight pulse. 2. Qi stagnation and blood stasis type The diagnostic basis for dysmenorrhea of Qi stagnation and blood stasis type is lower abdominal distension and pain during menstruation, which often occurs one or two days before menstruation. There may also be chest and breast distension, or scanty menstruation, or irregular menstruation, dark purple menstrual blood with clots, pain relief after the clots are discharged, and disappearance of pain after the menstruation is over, dark purple tongue or with petechiae, and a wiry or slippery pulse. 3. Damp-heat type Symptoms include lower abdominal pain before menstruation, refusing to be pressed, with a burning sensation, or accompanied by lumbar pain; lower abdominal pain at ordinary times, which is aggravated during menstruation. Low-grade fever fluctuates, dark red menstrual blood, thick and clotted, thick yellow vaginal discharge, short and yellow urine, red tongue, yellow and greasy coating, and stringy or weak pulse. 4. Qi and blood deficiency type The symptoms of dysmenorrhea caused by qi and blood deficiency are dull pain in the lower abdomen during menstruation, or a feeling of emptiness in the lower abdomen and genitals, a preference for massage, scanty menstruation, light-colored and thin menstruation, or fatigue, or a sallow complexion, or poor appetite and loose stools, a pale tongue, and a thready and weak pulse. I hope the above diagnostic basis for dysmenorrhea is helpful to you.
