2011 Chinese Medicine Practitioner: Ovulatory Functional Uterine Bleeding

Dysfunctional uterine bleeding (DUB), referred to as DUB, refers to abnormal uterine bleeding characterized by menstrual disorders due to dysfunction of the HPOU axis rather than organic lesions of the reproductive tract. Generally, patients with ovulatory DUB do not suffer from severe bleeding that affects their health.

1. Luteal insufficiency Small doses of androgens can stimulate the pituitary gland to secrete gonadotropin, promote follicle development, and thus improve corpus luteum function. Starting from the 5th day of the cycle, take 0.125-0.25 mg of diethylstilbestrol orally every night for 20 consecutive days, and use progesterone to supplement the deficiency in the body. Starting from the 20th day of the menstrual cycle, inject 10-20 mg of progesterone intramuscularly every day for 5-7 days. In addition to progesterone, chorionic gonadotropin can also be used for treatment. The purpose is to promote corpus luteum development and enhance corpus luteum secretion function. It can be started from the 15th to 17th day of the menstrual cycle (i.e. ovulation day), or from 2-3 days after the basal body temperature rises, intramuscular injection of hCG500-1000IU every day or every other day, for a total of 5 times. During the treatment process, vitamins C and E can be taken, which have a certain effect on the treatment of corpus luteum insufficiency.

2. The treatment of incomplete corpus luteum atrophy is not satisfactory. Curettage and hemostasis have a certain effect. Later, on the 21st to 25th day of each cycle, intramuscular injection of progesterone 10-20mg for 5 times, or oral administration of medroxyprogesterone 8-10mg, once a day, starting on the 18th day of the menstrual cycle, for 10 consecutive days. This can completely exfoliate the endometrium. Or try taking contraceptive pills to inhibit ovulation for 3 cycles, and observe the efficacy after stopping the medication.
