Dysmenorrhea is divided into two categories: primary dysmenorrhea and secondary dysmenorrhea. Primary dysmenorrhea refers to dysmenorrhea without organic lesions in the reproductive organs, accounting for more than 90% of dysmenorrhea, and secondary dysmenorrhea refers to dysmenorrhea caused by organic diseases of the pelvic cavity. 1. Through bimanual and trimanual examinations, some causes of dysmenorrhea can be found, such as uterine malformation, uterine fibroids, ovarian tumors, pelvic inflammatory disease, etc. Rectal examination can determine whether the uterosacral ligament is nodular and thickened, which is particularly important for the early diagnosis of endometriosis.

2. A history of recurrent pelvic inflammatory disease, irregular menstrual cycles, menorrhagia, intrauterine device placement, infertility, etc. is helpful in the diagnosis of secondary dysmenorrhea.

3. Hysteroscopic examination can detect small lesions missed during curettage, such as small myomas, polyps, ulcers, etc., and provide valuable diagnostic basis. It can be performed after diagnostic curettage.

4. Other examinations, such as erythrocyte sedimentation rate, leucorrhea bacterial culture, B-ultrasound pelvic scan, diagnostic curettage, and finally hysteroscopy and laparoscopy can identify the cause of dysmenorrhea as early as possible.

Warm reminder: Secondary dysmenorrhea is quite harmful, and most of them are caused by diseases, such as endometriosis, pelvic inflammatory disease, pelvic congestion, etc. In recent years, it has been found that when the endometrium synthesizes more prostaglandins, it can also cause dysmenorrhea. Therefore, it is necessary to determine the cause of dysmenorrhea through examination and treat it accordingly.
