Estrogen-progesterone replacement therapy for amenorrhea caused by premature ovarian failure

Estrogen-progesterone replacement therapy for amenorrhea caused by premature ovarian failure

Amenorrhea can be caused by premature ovarian failure and requires active treatment. Estrogen and progesterone replacement therapy for amenorrhea caused by premature ovarian failure is as follows:

Estrogen-progesterone replacement is suitable for young patients with premature ovarian failure. It can eliminate the symptoms of low estrogen and urogenital tract atrophy, and can also prevent long-term complications such as osteoporosis and Alzheimer's disease.

Long-term use of estrogen-progestin replacement therapy has certain risks and may lead to endometrial cancer and breast cancer. Studies have shown that when progestin is used for more than 10 days per month, estrogen-progestin replacement therapy can reduce the risk of endometrial cancer to almost zero, while the risk of breast cancer increases slightly, but the mortality rate does not increase. Usually, a sequential combination of estrogen and progestin is used.

Before using estrogen-progesterone replacement therapy, patients need to undergo necessary monitoring and follow-up. For patients with premature ovarian failure who do not have fertility requirements, transdermal estrogen patches can be used. There is no first-pass effect in the liver, which reduces damage to liver function, reduces the risk of endometrial cancer, and does not increase the risk of breast cancer. For POF patients who have had their uterus removed, only estrogen skin patches can be used without the use of progesterone.

Young patients with premature ovarian failure need more estrogen to maintain normal vaginal mucosa, so patients use estrogen preparations for the first 21 days and add progestin for the next 10 days.
