Early clinical manifestations of functional hypothalamic amenorrhea

Early clinical manifestations of functional hypothalamic amenorrhea

Functional hypothalamic amenorrhea refers to the abnormal release of gonadotropin by neuroendocrine cells in the arcuate nucleus of the central eminence of the hypothalamus, leading to anovulation and amenorrhea. The following four symptoms can be seen in the early stage:

Psychogenic amenorrhea

Patients with this type of amenorrhea often have a history of mental stimulation, oligomenorrhea and amenorrhea, and may also have infertility and weight loss. Examinations show high blood cortisol levels, but no related clinical symptoms; gonadotropin-releasing hormone stimulation tests show that the pituitary is responsive or unresponsive to exogenous gonadotropins

False pregnancy

Patients experience depression, amenorrhea, milk secretion, nausea, vomiting, loss of appetite and other early pregnancy-like reactions. It is a typical neuroendocrine disease. When patients think they are pregnant, the basal body temperature continues to be high and the prolactin level in the blood increases. However, when patients learn that they are not pregnant, the prolactin level may drop sharply, and menstruation may come.

Anorexia nervosa

Common in adolescent or young women, excessive dietary restrictions, vomiting, or even almost no eating. Obvious weight loss, at least 25% or even 40% less than the original weight. Patients have anorexia, stubborn refusal to eat, or other symptoms of dietary dysfunction. Amenorrhea is the most important manifestation, accompanied by varying degrees of sexual characteristics disappearance. Sexual dysfunction, menstrual disorders or complete amenorrhea.

Exercise-induced amenorrhea

It is common in female athletes. Due to long-term participation in intense sports training or competition activities, the hypothalamic-pituitary function becomes abnormal, causing delayed menarche or temporary menstrual disorders in normal menstruation and even amenorrhea.
