Ovarian cyst pedicle torsion is one of the more serious complications of ovarian cysts, with an incidence of about 10%. The conditions for pedicle torsion are that the tumor is large (generally refers to a diameter of more than 6 cm), the tumor pedicle is long, there is no adhesion to the surrounding tissue, and the tumor has a large range of movement in the abdominal cavity. Serous and mucinous ovarian cysts are most likely to have pedicle torsion, on the one hand because the tumor pedicles of these two types of tumors are longer, and on the other hand because their tumor center of gravity is mostly biased to one side, which is easily affected by strenuous exercise, sudden changes in body position and intestinal peristalsis. After the pedicle of an ovarian cyst is twisted, the venous return of the ovary will be blocked due to pressure, while the arterial blood supplying the ovary is still normal, causing the tumor to become excessively congested and purple-brown in color. As the disease progresses, the artery supplying the ovary also becomes blocked. Over time, the ovarian cyst will become necrotic due to ischemia. In severe cases, bleeding of varying degrees may occur, and even shock may occur. If the tumor pedicle is connected to the intestinal tract, it is easy to be infected with E. coli after the pedicle twists. The patient may have peritoneal irritation symptoms such as abdominal wall tension and infection symptoms such as abdominal pain and fever. The condition is critical and should be treated promptly. The patient will feel severe pain in the lower abdomen, some patients will vomit, and a very small number of patients will have a small amount of vaginal bleeding due to congestion of endometrial blood vessels. |
