Inhibit ovulation: If the patient is willing to control fertility, oral contraceptive tablets (compound norethisterone tablets or compound megestrol acetate tablets) are the first choice for the treatment of primary dysmenorrhea. More than 90% of symptoms can be relieved by oral contraceptives, which may be due to the inhibition of endometrial growth, reduced menstrual volume, and the reduction of pg volume below normal levels, resulting in weakened uterine activity.

The treatment can be tried for 3 to 4 cycles. If the effect is satisfactory, it can be continued. If the symptoms are not significantly improved, PGS synthesis inhibitors can be appropriately added. Since the medication must be taken throughout the menstrual cycle, and the effect only occurs 1 to 2 days at the end of the cycle, it is generally not popular with patients unless contraception is required at the same time.

3. Cottonpol: 20 mg of cottonpol acetate, taken once a day for 3 to 6 months, can achieve a 95% or higher efficacy in treating primary dysmenorrhea. However, it may cause obvious side effects, such as fatigue, palpitations, nausea, edema, dizziness, hot flashes, anorexia, osmotic diarrhea, etc. In severe cases, thrombocytopenia and hypokalemia may also occur.

4. β-receptor stimulants: by stimulating β-receptors on the muscle cell membrane, activating adenylate cyclase, and in turn increasing the intracellular cAMP content. On the one hand, it promotes the phosphorylation of sarcoplasmic reticulum membrane proteins and strengthens the binding of Ca++; on the other hand, it inhibits the activity of myosin light chain kinase, leading to uterine muscle relaxation and rapid relief of dysmenorrhea, but at the same time, it has the side effects of increasing heart rate and blood pressure.
