Treatment of premature menstrual irregularity by TCM differentiation of syndromes

Traditional Chinese medicine believes that premature menstrual irregularities are often caused by blood heat, heat disturbing the Chong and Ren meridians, or Qi deficiency that cannot control the blood, or kidney deficiency that makes the Chong and Ren meridians unstable. Those who suffer from blood heat should be distinguished between real heat and false heat.

Menstruation comes more than 20 days later, or even once every half a month, which is called premature menstruation. The mechanism of premature menstruation may be caused by heat in the liver meridian or deficiency of qi. It may also be accompanied by irritability, pain in the ribs and bitter taste in the mouth, dark red menstrual blood, thick and not light, red tongue and rapid pulse. This is because there is heat in the liver meridian, and the blood is not stored in the liver and is discharged prematurely. It may also be accompanied by palpitations.

Shortness of breath, lack of breath and laziness to speak, light red and sparse menstruation, pale and tender tongue, slow and weak pulse, that is, Qi deficiency cannot bind the pulse, the veins are loose, and Qi deficiency is not solid. Chinese medicine differentiation can be divided into the following 4 common syndromes:

Yang Sheng Blood Heat Type

The patient has early menstruation, heavy menstruation, bright red or purple color, accompanied by red face, irritability, thirst, red tongue, yellow fur, slippery pulse. The treatment should be to clear away heat and cool blood, and the prescriptions used are Qingjing Powder, Danzhi Xiaoyao Powder, etc.

Yin deficiency and blood heat type

The patient has early menstruation, little or even less menstrual flow, bright red and thick, accompanied by flushing, hot palms and soles, night sweats, restlessness and insomnia, dry mouth, red tongue with little or no coating, and a weak pulse. The treatment should be to nourish yin, clear heat and cool blood, and the prescriptions are Liangdi Decoction, Qinghua Drink, etc.

Qi deficiency type

The patient has early menstruation, with heavy and light-colored menstruation, clear and thin texture, accompanied by palpitations, shortness of breath, fatigue, pale complexion, loss of appetite, pale tongue with thin coating, and weak and thready pulse. The treatment should be to replenish qi and blood, with prescriptions such as Guipi Decoction and Buzhong Yiqi Decoction.

Kidney deficiency type

The patient has early menstruation, more or less menstrual volume, dark color, thin texture, accompanied by back pain, leg weakness, pale tongue, and weak pulse. The treatment should be to tonify the kidney and consolidate the Chong meridian, and the prescription is Guilu Bu Chong Tang.
