The theory of menstrual blood backflow implantation is currently recognized as an important cause of the disease. The shedding of endometrium with the blood is menstruation. If the shed endometrium flows back to the fallopian tube with the menstrual blood, and then flows into the pelvic cavity through the fallopian tube, implants on the ovary or the peritoneal surface of the pelvic cavity, continues to grow, and changes periodically under the action of ovarian hormones, it will form endometriosis lesions.

Uterine surgeries, such as cesarean sections in the second trimester and cesarean sections at term, may bring endometrial cells to the incision in the abdominal wall, causing endometriosis lesions at the wound. During childbirth, endometrial cells may also be brought to the episiotomy wound of the perineum, causing lesions in this area.

Surgeries within the uterine cavity, especially those that increase uterine cavity pressure, such as artificial abortion, ventilation, fluid insufflation, and angiography of the fallopian tubes, may press the endometrium into the pelvic cavity.

Certain congenital defects, acquired injuries or diseases may cause reproductive tract obstruction, resulting in the backflow of menstrual blood into the pelvic cavity, such as imperforate hymen, vaginal adhesions, occlusions, cervical adhesions and occlusions, etc.

Not paying enough attention to menstrual hygiene. Engaging in excessive and strenuous physical activities, such as strenuous gymnastics and dancing, during menstruation.
